WHO QUALIFIES FOR THIS PROGRAM
RESIDENCE
They must live in Berrien Co. Michigan or the surrounding area.
NO VISION CARE
They MUST NOT have insurance, Medicare, or Medicaid that covers vision care.
FEDERAL POVERTY
Their total household income MUST BE less than 200% of the Fed. poverty level.
APPROVED
If the patient meets the criteria and you can verify the first three, then send them our way.
