  1. I have read, understand, and agree with Vision Outreach Internationals Conflict of Interest policy.

  2. When a conflict of interest arises or when a potential conflict of interest emerges, I will disclose that conflict or potential conflict to the VOI Executive Committee or the VOI Board and seek a resolution of that issue.

  3. As of the date shown below, neither I, nor my immediate family have any substantial interest, nor act in any capacity which could result in a conflict of interest with the activities of Vision Outreach except as listed below:
