In order to complete your application, we will need some supporting documents. To comply with some state, national, or international regulations we will need you to scan or take a picture and upload the following items: (UPLOAD BELOW)
- Passport (current photo page)
- Current Medical License
- Medical School Diploma
- Residency Diploma
- Fellowship Certificate (if applicable)
- ABO Certificate
- Curriculum Vitae
- Immunization Records -We ask that you visit www.cdc.gov and view the traveler’s Health section for health information.
Denise is our MSICS TRAINING COORDINATOR. She will answer all of your questions and walk you through the process. If she doesn’t know an answer, she will work diligently on your behalf to find the answer.
